Poorly designed but hopefully a helpful survey. Consider using the additional comments to make one really important point per page...
My top three: edge traps created by longitudinal asphalt repair, effectively reduces lane width; too much highly reflective signage makes the roadway disappear; traffic light auto-sensors don't work well for motorcycles.
I didn't tell them about the school buses.
